Immigration Physical

Immigration physical exam is required by USCIS (U.S. Citizenship and Immigration Services) for specific individuals seeking permanent residence in the United States. This examination must be done by a medical doctor who is approved by USCIS to perform such physicals. After the performance of the immigration physical, if all the requirements are met, an I-693 form will be completed.

Dr. Kapil Passi, a board-certified Internal Medicine physician has been doing immigration physical since 2016. During the evaluation, he will determine the appropriate USCIS tests and vaccinations, which are required to complete the I 693 form.

What to bring and what to expect:

  • You can fill I 693 form online and print out a copy OR We will give you a current I-693 way to fill out at the office
  • Bring a government picture ID, such as a driver’s license, ID card, or passport.
  • Bring records of vaccinations with you, if you have them. If you do not have documents, it is not a problem – we will be able to prescribe vaccinations.

  • The labs are generally not covered, and we use CPL labs for all lab workup.
  • The doctor will provide all the forms for vaccinations and Lab workup.
  • Examination cost includes a physical exam and sealed I-693 form, as required by USCIS.
