Immigration physical exam is required by USCIS (U.S. Citizenship and Immigration Services) for specific individuals seeking permanent residence in the United States. This examination must be done by a medical doctor who is approved by USCIS to perform such physicals. After the performance of the immigration physical, if all the requirements are met, an I-693 form will be completed.
Dr. Kapil Passi, a board-certified Internal Medicine physician has been doing immigration physical since 2016. During the evaluation, he will determine the appropriate USCIS tests and vaccinations, which are required to complete the I 693 form.
What to bring and what to expect:
- You can fill I 693 form online and print out a copy OR We will give you a current I-693 way to fill out at the office
- Bring a government picture ID, such as a driver’s license, ID card, or passport.
- Bring records of vaccinations with you, if you have them. If you do not have documents, it is not a problem – we will be able to prescribe vaccinations.
