We are pleased to share that Priyanka Chaudhry, MD, FAHS, and Delana Brooke Jones, MPAS, PA-C are relocating their practice to a new private location in Coppell, effective February 1, 2026.
Please note: If you previously received a letter listing March 1, 2026, the correct relocation date is February 1, 2026.

Our new location is a private practice, which means the facility fee previously charged by Baylor has been eliminated, helping reduce your overall healthcare costs.

📌 New Patient Portal – Action Required
As part of our transition, we are now using a NEW patient portal that is separate from the Baylor system. This portal will be used for:
- Appointment confirmations and requests
- Secure messaging with our staff
- Forms, clinical updates, and visit information

If you did not receive login credentials, you may create an account using your mobile phone number by following the prompts.
🔹 Appointments:
All existing appointments from the Baylor system have been automatically transferred. Once logged in, please verify that your appointment date and time are correct.
If anything needs to be updated, send us a message through the portal under General Messages, and our team will respond through the portal or by phone.
Note: Until January 30, 2026, the portal may temporarily display the Baylor Headache Clinic address. This will automatically update after our final day at that location.
Referrals
If your insurance requires a referral, please contact your primary care provider to update the referral to our new Coppell address prior to your appointment.
